Health Risk Assessment available NOW

Better health and lower premiumsCompleting your Health Risk Assessment (HRA) is more convenient than ever. Instead of having only a few weeks to get it done, you now have months. You can complete the assessment anytime now through June 1.

The FOH Wellness Team and Blue Cross Blue Shield (BCBS) have partnered to offer this change. Their goal is to provide you more flexibility and time to complete the biometric screening requirement when it’s most convenient for you.

Participating in this screening each year is an easy way to lower your monthly insurance premiums. Doing so will earn you a $20 per month premium differential (a savings of $240 per year) as well as a gift from the Kansas City Fountain of Health (FOH).

Please note that the Biometric Screening and HRA events are only for active City of KCMO employees who are on the City’s BCBS medical insurance.

You will have from NOW to June 1, 2017 to complete the following:

  • Health Risk Assessment (HRA) – This 50-question, online self-assessment was previously offered only between May and June. It is available from NOW through June 1. This assessment can be found on your BCBS portal, MyBlueKC.com. You will need to log in using your BCBS username and password (this is not your City username and password).  If you did this assessment prior to January 1 or do it after June 1, it will not count toward your requirement.  
    • There will be limited access to technology to complete the HRA at a biometric screening event. Employees are encouraged to have the HRA completed prior to the screenings.
    • The FOH team will be setting up times at locations (field locations and KCFD stations) where computers will be available for employees to complete their HRA between now and June.
      • Employees may complete HRAs on any mobile device, computer or iPad that has internet capabilities and Google Chrome.
    • Follow these directions for completing your HRA:
  1. Visit MyBlueKC.com or download the Blue KC A Healthier You app. (Use Google Chrome)
  2. Enter your BCBS username and password (this is not your City password) and click LOG IN. If you are a first-time visitor, click REGISTER NOW. Be sure to have your member ID card (BCBS insurance card) available to reference. If you have forgotten your username and/or password, contact BCBS’s customer service at (816) 395-2121.
  3. Once logged in, click on A Healthier You from the “My Home” page.
  4. First-time users will be prompted to complete the onboarding personalization questions. This is not your HRA. This is simply 5-10 questions to prepare your dashboard.
  5. Click on the Take HRA Now, located in the bottom left-hand corner of your page. There will be 50 questions. Make sure you answer all 50.

By completing the HRA, you will fulfill one of the two required steps to receive the $20 per month premium differential.

  • Manual Submission Form (MSF) – Previously known as the Alternative Means Screening (AMS) Form. You can find this form on your BCBS portal. This form is available if you would rather have your physician complete your biometric screening instead of going to one of the events. To use the form, you will need to go into your BCBS portal at MyBlueKC.com and print your form. You will need your BCBS username and password to log in. (This is not your City username and password.) You will then take this form with you to your physician appointment or KCMO Health Care Trust Employee Clinic appointment and have your physician complete the entire form and fax the form and lab values to the number on the form.
    • If you complete the MSF Form with your provider and send it in to the number on the form between NOW and June 1, it will take the place of the screening event. You will still need to complete the HRA.
    • An automatic confirmation email will be sent to you within 48 hours to confirm acceptance of the completed form. If the form is missing information that requires action from you, you’ll receive an email specifying the missing information and requesting that you resubmit the form.

Biometric Screening events will still be offered in May and June.

Choose to Lose

The Choose to Lose Weight Management program is offered as part of the screening events. If you are utilizing the MSF Form, you will need to send Wellness Coordinator Jeanine Scrogum your lab results showing only your name, date of physical, and weight. We do not need to see any other biometric results. Gift cards for Choose to Lose will be given out at the screening events in May and June.
